Request for family register copies/extracts, resident's card copies and other certificates

Applications for family register copies/extracts, resident's card copies and other certificates are made at the following offices, and charges:

<Hours when certificates are issued>

Ward Office (Kuyakusho) /
Ward District Office(Kuyakusho Shutchojo)

Weekdays 9:00-17:30
· Not issued on Saturdays, Sundays and national holidays
· Until 17:15 only for persons applying for a domicile certification (koseki shomei) whose registered domicile (honseki) is not in this ward

· On Saturdays, Sundays and national holidays application only are received. (Certificates will be issued from the next Kuyakusho business day.)
· Applications for domicile registration (koseki) are received only until 17:15

*Applications for copies of residence registrations issued in a wider area beyond Osaka City may be made on weekdays between 9:00 and 17:00. Since no applications will be received outside these hours, persons with residence registrations in locations other than Osaka City must present applications to the registration desk by 17:00.

· Certification of registered domicile is required only of persons who are registered in Osaka City.

Family register copies/extracts
Register separation copies/extracts

Apply at: The resident registration sections of ward offices, branch offices, service counters (Umeda, Namba and Tennoji)
Charges: Family register copies/extracts = 450 yen per copy/extract
Register separation copies/extracts = 750 yen per copy/extract
Requisites: Seal of the person coming to the office/service counter. When an attorney is making the application, the power of attorney (unnecessary if the attorney belongs to the same family register as the applicant) and the seal of the attorney.

Copy of family register appendixes

Apply at: The resident registration sections of ward offices; branch offices, service counters (Umeda, Namba and Tennoji)
Charges: Copy of family register appendixes = 200 yen per copy
Requisites: Seal of the person coming to the office/service counter. When an attorney is making the application, the power of attorney (unnecessary if the attorney belongs to the same family register as the applicant) and the seal of the attorney.

Resident's card copy (incl. separation certificate)

Apply at: The resident registration sections of ward offices, branch offices, service counters (Umeda, Namba and Tennoji)
Charges: 200 yen per copy
Requisites: Seal (unnecessary when signed in person; sealing required when the signature is not by the applicant). Power of attorney is needed if an attorney is making the application.
When making an application for a certification of residence registration issued in a wider area beyond Osaka City, present your basic resident registration card or official photo ID.

*Only the applicant or a member of the same household may make application for a certification of residence registration issued in outlying areas. (Certification cannot be issued for a person whose name has been removed from the family register.)

Basic Resident Registration Card

Apply at: Resident registration section (jumin toroku-gakari) at the ward office (kuyakusho) where you are registered. (Application cannot be made at the service counter of a ward branch office [kuyakusho shutchojo].)
* No same-day issue service for basic resident registration cards.
* When applying for a photo ID basic resident registration card, you must present a photograph (4.5cm high x 3.5cm wide, no head covering, full-face, plain backdrop, taken within the last 6 months).
Charges: 500 yen each (for either photo or non-photo ID cards)
Card Issue: Your card issue notice will be sent to your address (together with an inquiry form). Please bring the following documents to the ward office where you made your application.
Residents possessing a driver’s license or passport with photo ID:
· card issue notice that you received from the ward office
· photo ID
· personal seal
Residents without photo ID:
· response form to the card issue notice together with the inquiry that you received from the ward office
· health insurance card (kenko hokensho), pension insurance booklet (nenkin techo), etc., for confirmation of identity
· personal seal

* Applications for Basic Residents' Register cards may also be made by post. For details inquire at the resident registration section of your ward office (kuyakusho jumin toroku-gakari).

Certificate of seal registration

Apply at: The resident registration sections of ward offices; branch offices; service counters (Umeda, Namba and Tennoji)
Charges: 250 yen per certificate
Requisites: Seal registration card

Certificate of facts of registration

Apply at: The resident registration sections of ward offices(foreign resident registration section in Ikuno Ward); branch offices; service counters (Umeda, Namba and Tennoji)
Charges: 200 yen per certificate
Requisites:: Power of attorney is required when an attorney is making the application (unnecessary if the attorney belongs to the same household as the applicant)

Request for copies, etc., by phone or mail
For a resident's card copy, certificate of facts on resident's cards, certificate of seal registration, certificate of facts of registration, reservation can be made by phone on Fridays. (the reservation by phone is not available if the following Saturday is a national holiday.)


If you make a reservation by phone at the resident registration section or the foreign resident registration section of the ward office of your registration between 9:00 and 16:00 of Friday (Thursday if the Friday is a national holiday), these documents can be received at the holiday office of the relevant ward office between 9:00 and 12:45 of the following Saturday.

Bring the seal registration card to receive the certificate of seal registration, an ID (either of driver's license, passport or certificate of alien registration) to receive the certificate of facts of registration.

You can make an application during after-hours of weekdays or on Saturdays, Sundays or National Holidays, and receive later by mail family register copies/extracts (incl. register separation), copies of resident's cards (incl. register separation), certificates of resident's card facts, or certificates of facts of registration.


Fill necessary information in the relevant application form at the holiday office of relevant ward office between 17:30 and 21:00 of weekdays or between 9:00 and 21:00 of Saturdays, Sundays or National Holidays, enclose charges and a return envelope (also available at the holiday office) with return stamp or stamp charge, and put it in the specified box. The certificate will be mailed to you later.

The person coming to the office must bring an ID the certificate, (either driver's license, passport or alien registration).
* When there is any specific request as to the information to be included in the certificate, the issuance is only available at the ward of registration.


Application can be made by mail for family register copies/extracts, register separation copies/extracts, and resident's card copies.


Fill an application form with necessary information and phone number accessible during the day, enclose a return envelope with an appropriate stamp and charges (use postal money order for the correct charge), and mail for application.

To view the Basic Resident Register, apply for viewing at the resident registration section of the ward office where the desired resident's card is registered or at branch offices of the ward.

Use family registers and resident's cards properly:

Osaka City Government hopes to build, the society in which basic human rights are well respected. The city thus promotes public awareness of the Dowa issue, inclusion of people with disabilities, women, and foreign residents, and other themes related to the protection of human rights, aiming to create a city cherishing the rights of each individual.
To protect citizens' human rights, respect their privacies and prohibit discrimination, the following measures are taken at ward offices, to ensure that resident's card copies will not be used illegitimately for discriminatory investigations, and that family registers and resident's cards will be used properly.

When applying for family register copies/extracts, etc.,

You must indicate the purposes of use and places of submission in the application form.
When the application is made by a person other than the applicant or a relative in the same family register as the applicant, a proof of the applicant's assent is required.
Even though the applicant him/herself is making the application, issuance may not be available depending on the purpose.

When applying for resident's card copies, etc.

You must indicate the purposes of use and places of submission in the application form.
Issuance may not be available depending on the purposes.
The relationship and the domicile of origin will be omitted from the copies issued, unless there are special requests, or if there are no proper necessities even though there are special requests.
You may be requested to apply for a certificate of resident's card facts, instead of a copy of resident's card.
When you ask a third person to apply for a copy, a proof of the applicant's assent and other documents are required.

When viewing resident's cards

You must indicate the purposes of use and places of submission in the application form.
Viewing may not be possible depending on the purposes, after a specific interview.
You must submit a pledge, and view with specified forms. The items you may read are the address, name of the person you requested, gender, and birth date.

Inquiry shall be made at the resident registration sections of ward offices, branch offices (Inquiry is obligatory)
