A woman who has reported that she is pregnant is issued a Mother and Child Health Record Book and annexed documents.
The booklet records the health condition of the pregnant woman and her infant. It also gives information on health and child-care.
When the Mother and Child Health Record Book is issued, a health nurse interviews the pregnant woman about her health condition, explaining about health examinations. |
Procedure |
After your pregnancy is confirmed, please immediately submit a Pregnancy Notification to the Health Section of the Health and Welfare Center in your ward. Please be sure to bring documents by which the My Number of the pregnant woman and her identity can be confirmed. The Mother and Child Health Record Book is then given to you.
When you move to or from Osaka City, within the same ward or to another municipality or change your name, please submit your Mother and Child Health Record Book and a notification of address/name change to the Health Section of the Health and Welfare Center in your ward.
