Home-visit Medical Service (訪問診断)

Doctors, nurses, caseworkers and other staff of Rehabilitation Center for the Disabled visit homes, medical institutions and welfare facilities together to meet persons with severe physical disabilities, incurable diseases etc. who have extreme difficulties in mobility and cannot leave their homes by car. The purpose of the visits is to assess severity of their disabilities - a process necessary for issuance of 身体障がい者手帳 (Shintaishogaisha Techo or Identification Booklet for a Person with a Physical Disability) - and, if needed, determine whether wheelchairs or assistive devices currently used are appropriate.

* Days of visit
In principle, the third and fourth Tuesday, and the second and third Thursday of every month.

* How to make an appointment
Please make an appointment at the Public Health and Welfare Department of the Public Health and Welfare Center in your local ward.

Visiting Assessment Service (出張判定)

People with physical disabilities can receive assessment to determine whether assistive devices currently used are appropriate at their local public facilities. Doctors, nurses, caseworkers and other staff of Rehabilitation Center for the Disabled are available for assessment at the ward offices and Public Health and Welfare Centers given below.

* Dates and locations of the visiting assessment service
The first Thursday: Minato ward office (Public Health and Welfare Center) (except when it falls on a national holiday)
The second Monday: Higashiyodogawa ward office (to be postponed to the following week etc. if it falls on a national holiday)
The third Tuesday: Joto ward office (Public Health and Welfare Center) (except when it falls on a national holiday)

Please make an appointment to schedule the date of assessment at the Public Health and Welfare Department of the Public Health and Welfare Center in your local ward.
