Identification Booklet for Person with Physical Disability
(身体障がい者手帳)

Eligibility (対象となる障がい)

Those with any one or more of the following disabilities:

• Visual impairment
• Hearing impairment and balance disorder
• Vocal impairment, speech impairment, and masticatory disturbance
• Orthopedic impairment
• Impairment of internal organs (cardiac disorder, renal disorder, respiratory disorder, bladder or rectal disorder, intestinal disorder, and immune dysfunction, dysfunction of the liver)

Degree of Disability (障がいの程度)

isabilities are classified into six grades, grade 1 to 6, according to severity.

Where to Apply (交付申請)

To have an Identification Booklet for a Person with a Physical Disability issued, please apply at the Welfare Department of the Public Health and Welfare Center in your local ward. The following are necessary for making an application:

• The 身体障がい者手帳交付申請書 (Shintaishogaisha Techo Kofu Shinseisho or Application Form for Issuance of an Identification Booklet for a Person with a Physical Disability) (Your seal should be imprinted on the form.)
• The 身体障がい者診断書・意見書 (Shintaishogaisha Shindansho/Ikensho or Medical Certificate/Opinion on Physical Disability) (This form should be prepared by a doctor registered under Article 15 of the Act on Welfare of People with Physical Disabilities.)
• A portrait of the applicant (4 cm in length x 3 cm in width)

Reissuance (再交付申請)

Please apply for reissuance of the Identification Booklet for a Person with a Physical Disability at the Welfare Department of the Public Health and Welfare Center in your local ward if: there has been some changes in the severity of your disability; you are suffering from an additional disability; or if your identification booklet has been lost or damaged.

Changes of Address or Name (居住地・氏名の変更)

In case of change in address, please notify the change to the Welfare Department of the Public Health and Welfare Center of the ward where the new address is located (or to your nearest welfare office, if the new address is not in Osaka City). The current Identification Booklet for Person with Physical Disability and your seal are necessary to the procedure to change address.

Return of an Identification Booklet (返還)

If a holder of Identification Booklet for Person with Physical Disability died, please notify his/her death to the Welfare Department of the Public Health and Welfare Center in the local ward. The identification booklet and the seal of the person who notify are necessary.
