Where to apply (申請窓口)

Please apply at the Welfare Department of the Public Health and Welfare Center in your local ward.
(Following receipt of the application, the institution in charge examines the disability of the applicant to determine its severity.)

Examining institutions (判定機関)

Those aged below 18 years: Osaka Central Child Guidance Center
Those aged 18 years or older: Heartful Plaza (Counseling section of Osaka Municipal Rehabilitation Center for People with Disabilities)

Degree of Disability (障がいの程度)

A: Severe
B1: Moderate
B2: Light

Procedures to be Completed (手続きの方法)

First-time application (新規申請)

Application should be made by the applicant in person or by his/her guardian at the Welfare Department of the Public Health and Welfare Center in the local ward. Please be sure to bring your photo (upper body, 4 cm in length and 3 cm in width) and your seal when filing an application.

Examination for re-judging the severity of disability (再判定)

When the Identification Booklet for a Person with an Intellectual Disability is issued, the month of the next examination for re-judging severity of disability is fixed. By the month of the next examination, please be sure to bring the examination either at the Osaka Child Guidance Center (for those aged below 18 years) or at the Welfare Department of the Public Health and Welfare Center in the local ward (for those aged 18 years or older).

Reissuance and change (再交付・変更)

If the booklet is lost or damaged, or there are changes in name and others, please notify the Welfare Department of the Public Health and Welfare Center in the local ward.

Changes of address (居住地の変更)

In case of a change in address, please notify the change to the Welfare Department of the Public Health and Welfare Center in the ward where the new address is located (or to the nearest welfare office if the new address is not in Osaka City). The current Identification Booklet for a Person with an Intellectual Disability is necessary to the procedure to change.

Return of identification booklet (返還)

If Identification Booklet for a Person with an Intellectual Disability has become unnecessary because of the death of a holder or moderation of disability to the extent that the services are not available, please notify the Welfare Department of the Public Health and Welfare Center in the local ward. The identification booklet and the seal of the person who notify are necessary.
