Calculating Your Premium (保険料の計算方法)

‘National Health Insurance Premium’ = ‘Premium for Medical Insurance + ‘Premium for supporting the Medical Care System for the Elderly Aged 75 and Over’ + ‘Premium for Long-Term Care Insurance’ (* only household in which person aged 40 to 64 includes).
Annual amount of insurance premium for April to next March is determined in June, and 国民健康保険料決定通知書 (Kokumin Kenko Hokenryo Kettei Tsuchisho or Notice of Assessment of Premium for National Health Insurance) is sent to you. If you enroll in between June and next March, the insurance premium calculated based on the month you enrolled will be notified.

  • You should pay the premium of the month when you enroll in, but need not to pay the premium of the month you discontinue.
  • You are required to pay your insurance premiums for the year in 10 installments from June to March next year.
  • When you become 75 years old in the middle of the fiscal year, you have to pay the insurance premium from June to the previous month of your birthday. In case there is another insured person in the same household, you should pay equally-divided amount of total premium (for the year when you are 74 years old) every month during the period between the following month of your birthday and next March, in order to ease the out-of-pocket insurance premium per month of your household.
  • When you become 40 years old in the middle of the fiscal year, you are requested to pay the Long-Term Care Insurance premium from the month of your birthday (previous month, if the first day of the month is a birthday), and to pay equally-divided amount of the total premium every month during the period between the following month of your birthday month (previous month, if the first day of the month is a birthday) and next March. If your birthday month (previous month, if the first day of the month is your birthday) is any month after June, you can start to pay Long-Term Care Insurance premium from the month if requested after your birthday.
  • When you become 65 years old in the middle of the fiscal year, you have to pay Long-Term Care Insurance premium from June to the previous month of your birthday month. (Two months earlier, if your birthday is the first day of the month), and you should pay equally-divided amount of the total premium every month during the period between the following month of your birthday month and next March, in order to ease the out-of-pocket insurance premium per month in your household.

Reduction or Exemption of Premiums (保険料の減額・減免)

  • Reduction of premiums under Japanese law
    If your total household income earned in Japan during the previous year is below the prescribed amount, the National Health Insurance premium is reduced by 70%, 50%, 30% (to be applied), or 20%, depending on your income level.
  • Reduction or exemption of premiums under Osaka City's program
    You may be eligible for reduction or exemption of the health insurance premium if you have difficulty in paying the premium due to a considerable decrease in income resulting from: bankruptcy; layoffs; retirement; discontinuance of business; a three-month or longer cessation from business; business slowdown; illness of head of household; or other reasonable causes and due to earthquakes, fire, windstorms, floods and other disasters. If any of these cases applies, consult with the group in charge of health insurance and pension programs at your local ward office.
    Exemption application shall be made, however, seven (7) days prior to the due date of the month in which you would like to receive the exemption.

You need to declare your income to the group in charge of health insurance and pension programs at your local ward office, if you have not done so. Without your declaration, your premium cannot be calculated correctly and the decision of reduction and exemption cannot be made.
