Requesting Taxation Certificates Postal Delivery（郵便での税証明書の請求方法）

You may request postal delivery of the approved Taxation Certificates.
市税に関する証明書を郵便で請求することができます。

1　Required items　用意するもの

（1）Application Form　申請書

Please complete all required information on the Application Form.
申請書にすべての項目を記載してください。

Please make sure to fill in your telephone number available in the daytime.
日中ご連絡のつきやすいお電話番号の記載をお願いいたします。

（2）Service Charge　手数料

Please purchase "Teigaku Kogawase" at postal office, and enclose with application package.
(Teigaku Kogawase: Fixed amount postal money order)
郵便局で定額小為替を購入し、同封してください。

Taxation & Income Certificate　課税証明書

It costs 300 yen per a year.
1年度につき300円です。

Tax Payment Certificate　納税証明書 / Property Valuation Certificate　固定資産評価証明書

It costs 300 yen each certificate.
1年度1税目または1件につき300円です。

（3）Return Envelope　返信用封筒

Please make sure to enclose the return envelope printed with your address, name, and with post stamp attached.
返信用封筒を同封してください。ご住所、お名前をご記入いただき、切手を貼ってください。

（4）Identification　身分証明書

If present address is different from address after Osaka city, please attach the copy of your identify card.
現在の住所が大阪市から最初に転居した住所と異なる場合、身分証明書のコピーを添付してください。

2　Sending the application package　送付方法

Please include (1) the Application Form, (2) Service Charge, (3) Return Envelope and (4) your Identification Copy in an envelope.
申請書、定額小為替、返信用封筒、身分証明書のコピーを封筒に入れてください。
